Apply for a Grant

The SCCMA Foundation is proud to offer community grants that support our mission to advance physician wellness, promote health equity, and foster innovative approaches to care in Santa Clara County.

Our grant program is designed to fund impactful, local projects that align with one of our two core focus areas:

Grant Focus Areas

1. Physician Wellness

We support programs that address physician burnout, moral injury, and mental health by providing:

  • Peer support or counseling opportunities

  • Wellness and leadership workshops

  • Resources or initiatives that prioritize physician well-being

2. Community Health

We fund projects that strengthen ties between healthcare and the community, with a focus on:

  • Culturally responsive health education and outreach

  • Public health efforts in underserved communities

  • Initiatives that promote equity, access, and trust

  • Medical education programs or leadership development


Grant Levels Available

Applicants may apply for funding at one of the following levels:

  • $1,000

  • $2,500

  • $5,000

Grants are available to local nonprofits, community groups, healthcare organizations, or physician-led initiatives working within Santa Clara County.


Who Should Apply?

We welcome applications from:

  • Community-based organizations

  • Health and wellness coalitions

  • Physician-led groups

  • Nonprofit clinics or outreach programs

  • Medical education or training programs

Whether you’re addressing mental health among healthcare providers or bringing health education to an underserved neighborhood, we want to hear from you.


How to Apply

To apply, simply complete our online Grant Application Form: HERE

You’ll be asked to provide:

  • Basic organizational and contact information

  • A short project summary and budget

  • Target audience and timeline

  • Description of how the project aligns with our mission

  • Optional supporting documents such as 501(c)(3) letter or letters of support

Post-Grant Requirements

All grantees are required to submit a brief follow-up report within 60 days of completing their project. This helps us highlight the impact of your work and share your success with the broader SCCMA community.

Your report should include:

  • A summary of outcomes

  • Lessons learned or testimonials

  • At least one photo or visual (with proper permissions)

Select projects may be featured on our website, social media, or in our newsletter.


Grant Cycle

We review applications on a rolling basis unless otherwise specified. Please allow 4–6 weeks for review and notification.
