Beginning June 1, 2023, the Medical Board of California (Board) will no longer accept or process paper-based applications for a Physician’s and Surgeon’s (P&S) License. The Board will remove the paper application from the Board’s website on May 19, 2023.

The Board began transitioning to a paperless application process on February 1, 2023, when it stopped accepting and processing paper-based applications for Postgraduate Training Licenses (PTL) and the application to transition from a PTL to a P&S License.

The transition to a paperless application process helps both applicants and the Board by increasing the efficiency of the review process, enhancing security, and reducing costs and impact to the environment.

Except under limited circumstances, paper applications submitted on or after June 1, 2023, will not be processed and the Board will notify the applicant they must apply through the BreEZe online system

To apply online, you will need either a U.S. Social Security Number (U.S. SSN) or Individual Taxpayer Identification Number (ITIN). If you do not have a U.S. SSN or ITIN, please email the Board at Licensing.Questions@mbc.ca.gov to request the Physician’s and Surgeon’s License Application be emailed or mailed to you. You will be required to submit the application and fees by mail only.

If your license has been cancelled and you are reapplying for a Physician’s and Surgeon’s License, please email the Board at Licensing.Questions@mbc.ca.gov to request the Physician’s and Surgeon’s License Application be emailed or mailed to you. You will be required to submit the application and fees by mail only.
